Re-Entry to Practice License

Applicants that previously held a registration with the College, but surrendered for a period of time, will need to re-apply for registration. 

Jurisprudence Exam

Applicants are required to successfully complete a jurispurdence exam. The exam will be assigned to applicants as per the REG.032 Jurisprudence Exam policy.

Refer to requirements in REG.006 Re-Entry Practice License (found under registration policies)

Acknowledgement of Supervision Form

If required, complete and sign before commencement of employment . Once completed please upload to your supporting documents. Please review registration policy PGL.010 Guidelines for MLP Practising Under or Providing Supervision or Direction.
